Product Repair Request Form

Before submitting a repair request using this form, please reach out to our team via email or your lab’s private SpikeGadgets support Slack channel. We will be happy to help troubleshoot any issues encountered. This helps ensure that we have all the information necessary to repair your device as quickly as possible.

To submit a repair request, please proceed with the following steps:

  1. Complete the below form.
  2. Print and fill out the Product Repair Shipping form to be included with your shipment.
  3. Ship your repair to:

    Please use a shipping service with a tracking number and insurance for the full value of the included items. When shipping internationally, we recommend shipping with FedEx. Disclaimer: SpikeGadgets is not liable for the loss or damage of any incoming packages.

  4. Please enter your shipment tracking number below or email it to support@spikegadgets.com.

NOTE: Product repair assessment is always free, but shipping and parts & labor costs may be associated with product repairs beyond the Spikegadgets standard 2-year warranty. The user will always receive a repair cost estimate for out-of-warranty repairs before repairs are carried out.
